This DLC adds a whole new story mode with 12 missions, several new NPCs, some new buildings and most importantly, new weapons and mechanics. To me, the standout additions are the flamethrower and Fishman (the henchmen introducing said flamethrower). The new weapons, mainly flamethrower and machine gun, add a lot to the tactical combat by allowing you to lock down areas and luring hostiles into the flames. It is also worth mentioning that there are at least 2 unique versions of the new weapons and you don't have to pick between your boss and the 'specialized' henchmen when it comes to distributing them.
On the city map, rival gangs also change things big time. The AI can be very aggressive and keeps you on your toes more than the cops ever did. With the new building, you can also drive sports cars and armored cars, saving you the time to search nice vehicles on the map.
For all of these reasons, I recommend this DLC. However, I have some personal pet peeves that I want to mention. While the missions are mostly fun, the story in itself is not. I want to build a criminal empire, roast my enemies in the cleansing flames of my flamethrower and marry Charlie Luciano. Not retire from crime for a moody, nagging waifu with a rose petal spamming katana. It just doesn't belong into a gangster game for me. I briefly had hope to get one of the cool Japanese hitmen as henchman, but then the waifu was forced onto me and that hope died a slow, painful death. By 'forced onto me', I mean a mechanic I absolutely loathe in party based games - she blocks an active gang slot for most of the story, if I like it or not. You may have guessed it, I do not and would rather have had the freedom to compose my gang however I want. To be fair, you get one more active slot, but it still feels like I'm gimped by only using the other henchmen.
To summarize: great new mechanics for both city and tactical, new weapons, sports cars, 12 new missions and maps, great new henchmen - except the one that seriously gets on my nerves (mechanically and story-wise).
